This form is to collect information about children that are interested in taking part in HAF and/or HAP activities.
Within the soft launch from 16th June to 27th June inclusive the Alliance are able to book two activities for each child registered with us.
We will contact you to book these activities during that two weeks. If you do not respond we will be unable to book on your behalf.
Please note that you must have registered with the new system (details below) for us to book your child(ren) on to activities.
We are able to discuss additional support or transport if required.
We will also use this information to look at the provision currently available and make recommendations on how this provision can be made inclusive for disabled children who need support to access these activities.
Your input is valuable to evidence the need for additional support to be available and remove barriers.
