This form is to collect information about children that are interested in taking part in HAF activities, but may be currently unable to due to support or access needs. We will use this information to look at the provision currently available and make recommendations on how this provision can be made inclusive for disabled children who need support to access these activities. We will endeavour to match an activity with the appropriate support, however this may not be possible at this time. Our aim is to provide provision for some children in the holidays. However your input is valuable to evidence the need for additional support to be available and remove barriers. Due to this, completion of this form will not guarantee an activity being offered.
