Gloucestershire Families Support Request Form
Please complete this form to share your concerns and one of our Advisers will be in touch as soon as possible. We have a large number of families requesting help and, as our Advocates and Family Support workers are Carers themselves, there may be a delay in replying to your request.
Once we are able to respond to your request, providing that you live in Gloucestershire, we will contact you and may arrange a free consultation of up to 30 minutes. Hopefully we will be able to provide the advice that you need in this session but, if your issues are complex or require more time, then you may be referred for up to a further 4.5 hours of additional support.
This support might include individual support for your family, help preparing for and attending meetings with any agency supporting your child (assessments, reviews, planning meetings, child protection conferences, EHCP reviews, health care planning meetings etc); supporting you to make effective complaints; facilitating meetings between you and professionals to ensure effective communication; helping you through complex procedures, writing letters, putting across their views etc
You will receive an email copy of this form once you have submitted it and we aim to be in touch to explain how we might help within 5 working days.

Please give us some details of your concerns. Please include as much detail as you can about why you need support and we will arrange for someone with the relevant skills to get in touch with you to find out more.
It would be useful to know what additional needs your child/young person has, what service(s) (e.g. education, social care etc.) are of concern and the type of issues that you have - failure to discuss/assess needs, failure to provide agreed needs, etc.

The Parent and Carer Alliance C.I.C. is only able to get funding to provide advice and support through Grant funders, such as the National Lottery Community Fund.
For funding to continue to be given, the Alliance needs to demonstrate how the support we provide helps families like yours.
We need YOUR HELP and FEEDBACK to do this, as telling us what worked for you, helps funders understand why these services are needed.
