Event Feedback Form

We hope our Advice Service has helped you. Please complete this form to share your experiences of the service we provide. Any free or reduced cost advice has only been possible due to funding from the national lottery community fund, and Barnwood Trust.​

 

We need YOUR HELP to be able to continue to be able to offer such services. Your feedback is VITAL is evidencing the impact our support service is having on families and evidencing the need of this service, to our funders.​

 

It also will help us make long term improvements and all suggestions are welcome.​

 

Families who have received support from our service, but who do not fill out a feedback form, will not be considered for further support if needed in the future. This is due to the importance the feedback has on evidencing the work we are doing and without the feedback, we would not be able to secure grants to make sure this work can continue.

  • To request feedback from you about what you think about any service or support provided to you by the Parent and Carer Alliance  e.g. feedback to the National Lottery of the impact of advice and support that you have received as a direct result of their providing funding, which may involve them contacting you.

  • To track information about the use of our website including information obtained via cookies to develop the website.
